A woman looking to regulate her cycles has different options than one who wishes to conceive or improve her metabolic profile. These distinctions are at the heart of the question many patients ask themselves: is this really the right treatment for my situation?

POMS management does not follow a single protocol. It is built around the patient's primary goal, her phenotype, and her tolerance profile.
Regulating cycles and reducing hyperandrogenism
For women not currently planning a pregnancy, there are three main medication options.
Combined oral contraceptives are prescribed as a first-line treatment. They regulate cycles and reduce the manifestations of hyperandrogenism (acne, hirsutism). They do not treat the underlying cause of POMS but alleviate symptoms while being taken.
Spironolactone, prescribed for its anti-androgen effect (off-label for this indication), is a well-tolerated alternative. It can be offered alone or in combination, but is contraindicated if pregnancy is desired.
Cyproterone acetate has a more pronounced anti-androgen effect. The ANSM has issued an alert regarding a low but documented risk of meningioma associated with prolonged use: brain MRI monitoring is recommended for patients on long-term treatment.
Improve metabolic profile
Metformin is an insulin sensitizer originally developed for type 2 diabetes, used in POMS with a dominant metabolic component. It can help improve insulin resistance and, in some patients, regulate cycles.
Tirzepatide (Mounjaro) is approved by the May 2026 HAS guidelines for patients with POMS and a BMI ≥ 30. It acts on GLP-1 and GIP receptors, impacting the insulin resistance characteristic of POMS with a dominant metabolic profile.

Promoting fertility with POMS
Ovulation induction options are offered progressively according to the 2023 ESHRE/Monash guidelines:
- Letrozole : recommended as a first-line treatment, it stimulates ovulation with a good safety profile
- Clomiphene : an alternative to letrozole, still used in some practices
- Gonadotropins (injectable ovarian stimulation): second-line treatment, requiring close medical monitoring
