Most aortic aneurysms cause no pain or noticeable discomfort, especially since they can develop for a very long time without any clinical manifestations. For many patients, they are discovered during an examination performed for another reason. However, this condition deserves attention: certain signs, even subtle ones, and specific risk profiles warrant an assessment, regardless of whether symptoms are present. The onset of symptoms, however, must be considered a surgical emergency requiring the fastest possible care.

The absence of symptoms does not mean the absence of risk. Certain profiles warrant screening via Doppler ultrasound, regardless of any complaints. The French National Authority for Health (HAS) recommends this screening for men over 65 who have smoked, as well as for individuals with a family history of aortic aneurysm. Outside of these situations, an aneurysm may remain undiagnosed in the absence of targeted imaging.

When an abdominal aortic aneurysm causes symptoms, they most often include:
- Abdominal, lower back, or flank pain, which can sometimes be diffuse and difficult to pinpoint
- A sensation of throbbing or pulsing in the abdomen
- A pulsating mass that can be felt, usually in or above the navel area
This pain can be mistaken for common lower back pain or kidney pain. In patients with vascular risk factors (age, smoking, hypertension), aortic imaging can help clarify the source of the pain.
For athoracic aortic aneurysm, symptoms, when present, may include:
- Chest or back pain, located between the shoulder blades
- Difficulty breathing due to compression of surrounding structures
- Hoarseness or difficulty swallowing, if the recurrent laryngeal nerve or esophagus is compressed

Warning signs that require immediate medical attention
Sudden, intense pain in the abdomen, back, or groin in someone with a known or suspected aneurysm is a serious medical warning sign. It may be accompanied by low blood pressure, cold sweats, marked paleness, or loss of consciousness. These signs suggest an aneurysm is leaking or has ruptured, which is a life-threatening medical emergency.
