Diagnosing an aortic aneurysm: from screening to a full assessment

Tests used at each stage of the process

Three tests are used in the diagnostic process for an aortic aneurysm, each at a different stage of care.

Aorto-iliac Doppler ultrasound is the screening and monitoring test. It measures the diameter of the aorta, requires no injections, and can be performed in a doctor's office. For small aneurysms, it is the primary tool for long-term follow-up: its frequency is adjusted based on the measured diameter and increases as the aneurysm approaches the threshold for intervention.

Aorto-iliac CT angiography is the gold-standard test as soon as a surgical decision is being considered. It produces three-dimensional imaging of the aorta and adjacent arteries, with precise measurements of the vessel's diameter, length, and morphology. This data, collectively known as "sizing," directly determines the choice of surgical technique and the dimensions of the endoprosthesis.

MR angiography is an alternative to CT angiography for patients with renal insufficiency or a contraindication to iodinated contrast agents. It provides comparable anatomical information without radiation exposure.

Profiles eligible for targeted screening in France

Since most abdominal aortic aneurysms are asymptomatic, certain profiles warrant screening even before symptoms appear. French guidelines identify three specific situations:

• Men over 65 who have a history of smoking : The HAS recommends a one-time Doppler ultrasound screening for this group, due to a significantly higher prevalence than in the general population. ‍
• Family history of aortic aneurysm (father or brother affected): repeated screening is recommended, regardless of age or smoking status. ‍
• Arterial aneurysm in another location (popliteal or femoral artery): the presence of an aneurysm in another vessel requires screening for a potential associated abdominal aortic aneurysm.

In these situations, the primary care physician will refer the patient to the appropriate specialist for a targeted assessment.

Is your assessment sufficient to make a decision?

A Doppler ultrasound can detect an aneurysm and monitor its progression. However, it does not provide the anatomical measurements required for surgical planning. Before placing an endograft, the vascular surgeon requires a CT angiogram with "sizing": exact diameter, aortic neck morphology, and the angulation and caliber of the iliac arteries. This exam also allows for a three-dimensional characterization of the aneurysm's anatomy and its relationship with the digestive and renal arteries. These data directly determine the technical feasibility of the endograft and the dimensions of the prosthesis to be used.

Beyond aortic imaging, a complete preoperative assessment includes a cardiac evaluation (to estimate surgical risk), an assessment of renal function, and a respiratory workup. This comprehensive assessment is just as critical to the decision to operate as the diameter of the aneurysm itself.

The question "are the tests I've had sufficient to make a decision?" is one of the most legitimate reasons to seek a second opinion. A specialized vascular surgeon can verify whether the available data are sufficient to make an informed decision and identify any additional tests that may be necessary before proceeding further.
